Continuum Hospice Care
Our whole family was astonished at the extraordinary helpfulness,
caring and kindness of every one of your staff.
Continuum Hospice Care provides the best in specialized care for people in the final stage of life. The ultimate goal of hospice is to ensure that our patients' values, beliefs, lifestyle, and culture are respected, and that each patient's choices are honored.
A holistic approach to patient care hospice:
Assists with life's responsibilities
Enhances our patients' physical, emotional and spiritual comfort
Supports our patients' loved ones
Ultimately, we make it possible for patients and caregivers to use this final stage of life for review, closure, and even personal development.
With the help of hospice, my father was able to go on living as he always had:
sharing small moments with those he loved.
Our patients may be at home, in the hospital, in a hospice residence, or in a nursing home. And patients' own physicians, families, and others important to them are encouraged to participate in their care.
Nonsectarian, not-for-profit, Continuum Hospice Care provides comprehensive hospice care without regard to race, creed, disease, sexual orientation, or ethnic background. Licensed and Medicare Certified in 1988 as Jacob Perlow Hospice, we have cared for over 20,000 patients and families in Manhattan, Brooklyn, Queens and the Bronx.

The Grieving Center One Year Anniversary
This month is the first year anniversary of the Grieving Center. We would like to thank the many people who have shared their stories of grief. Over the past year we have produced 18 segments on a wide variety of subjects for a total of nearly three hours of programming. In our second year we have already filmed a number of powerful programs, which will be posted over the next few months. We are also introducing The Grieving Center Satellite Program: The program is a way for sponsors to benefit from the ever increasing traffic and audience the Grieving Center receives. The program offers a sponsor a fully produced video, the ability to post their own videos and their own dedicated discussion/forum group. If you are interested in knowing more about this program please contract Eric Marciano at em@grievingcenter.org

"When Someone Dies: Understanding Grief"
Nothing is more overwhelming than suddenly facing the reality of death. Dealing with the passing of a family member, friend or school acquaintance can be devastating for young people. Many adolescents resist the concept that grief is a normal or necessary emotion. This video is a resource with information that will teach audiences that death and grieving are natural, inevitable and manageable parts of life.
